COVID-19 Protocols and Vaccination Requirements

All attendees, presenters, exhibitors, staff, volunteers, and vendors attending the 102nd NCSS Annual Conference, December 2-4, 2022 must provide proof of vaccination for admittance. 

NCSS is working with CrowdPass, a HIPAA-compliant service, to verify vaccinations. It should only take you a few minutes to complete a simple, secure registration, and upload process. CrowdPass follows a thorough two-step verification process. Most verifications are completed within an hour or two, and CrowdPass will send a confirmation within one business day. CrowdPass will accept vaccination status approved by the World Health Organization (WHO) and the Centers for Disease Control (CDC). As part of the approval process, you must also accept and sign both the NCSS Attendee Waiver and Code of Conduct form, and the CrowdPass waiver.

CrowdPass Instructions

You will need your vaccination card on hand (or a copy/photo of it saved on your computer or mobile device) so you can reference the information on it, to take or upload a picture. Accepted file formats for your upload are .png, .jpg, .jpeg, or .pdf.

  1. To begin, click on the NCSS Vaccination Verification Form to log into CrowdPass with your Google or Facebook credentials, or you can create an account with your email address. (You may receive an email asking you to confirm your email address.) Click confirm to proceed with the verification process.
  2. Provide some brief contact information using your name exactly as it appears on your vaccination card, agree to the NCSS Waiver and Code of Conduct form and the CrowdPass waiver, then answer yes to the question “Are you fully vaccinated?”
  3. Fill in the information from your vaccination card, indicating which vaccine you received and when you received each dose. (If you received your vaccination outside of the United States, select "other". CrowdPass will verify international vaccine cards, but it may take up to 12 hours to receive your email with a verification code. Non-standard U.S. vaccination cards may also take additional time to be approved.)
  4. Take or upload a photo of your vaccination card and click submit.
  5. You will receive a confirmation email from CrowdPass that contains your vaccination verification code. Your vaccination verification code is unique to you and may not be shared or reused. Most people receive their confirmation email within a couple hours; however, it may take up to 12 hours during busy times. Be sure to check your junk or spam folder if you do not receive this message.

Exemptions

Exceptions will be made on a case-by-case basis only for those who cannot receive the vaccine due to a medical condition. 

To submit a request for an exemption, attendees will have to submit a request with the reason for the exemption to conference@ncss.org.  

If approved, attendees will be required to provide proof of a negative COVID-19 test result within 48 hours of your arrival. You must upload your test result to CrowdPass.

NCSS Waiver and Code of Conduct Acknowledgement

I acknowledge that travel to and attending in-person events involves the risk of contracting communicable disease. I agree to comply with all safety protocols posted by NCSS at www.socialstudies.org/conference or announced by the conference facility.

I agree to forever release, covenant not to sue, waive and hold NCSS and its employees, officers, directors, and other representatives harmless from and against all liability, claims, actions, damages, or expenses arising from the contraction or spread of communicable disease or enforcement of safety protocols at the NCSS Annual Conference.

I agree to comply with the NCSS Code of Conduct at the NCSS Annual Conference. I authorize NCSS and its representatives to take photos or video of me while at the Annual Conference, to identify me by name, and to disseminate the photos and video for NCSS purposes.

Further, I understand and agree that NCSS may share any COVID-related information about me that NCSS receives as part of such health and safety protocols with public health authorities or other regulatory agencies, as required by applicable law.

CrowdPass Waiver

Liability Waiver And Release Of Claims
In consideration of being permitted to attend the event or activity in connection with CrowdPass’s services (“Activity”), and in recognition of CrowdPass’s reliance hereon, I agree to all the terms and conditions set forth in this agreement (this “Release”).

1. Representations

a. I confirm that I am: (a) in good health, in proper physical condition, and do not have any medical or other conditions that would impair my ability to participate in the Activity; and (b) not experiencing symptoms of the novel coronavirus COVID-19 (“COVID-19”), do not have a confirmed or suspected case of an infectious disease.

b. I will comply with all federal, state, and local laws, orders, directives, and guidelines related to the Activity and COVID-19 while participating in the Activity. I will also follow all instructions, recommendations, and cautions of CrowdPass and the event organizer at all times during the Activity. If at any time I believe conditions to be unsafe, that I am no longer in proper physical condition to participate in the Activity, or I begin experiencing symptoms of COVID-19, I will immediately discontinue further participation in the Activity.

2. Assumption of Risk

a. I am aware of the contagious nature of infectious disease, including but not limited to COVID-19, and the risk that I may be exposed to or COVID-19 by engaging in the Activity, which may result in serious illness, personal injury, disability, and/or death.

b. I understand that the CrowdPass nor the event organizer cannot guarantee that I will not be injured or become infected COVID-19 due to my participation in the Activity and that engaging in the Activity may increase my risk of contracting COVID-19.

c. NOTWITHSTANDING THESE RISKS, I ACKNOWLEDGE THAT I AM VOLUNTARILY PARTICIPATING IN THE ACTIVITY WITH KNOWLEDGE OF THE DANGERS INVOLVED. I HEREBY AGREE TO ACCEPT AND ASSUME ALL RISKS OF INJURY, ILLNESS, DISABILITY, DEATH, AND/OR PROPERTY DAMAGE ARISING FROM MY ENGAGING IN THE ACTIVITY.

3. Release

a. I HEREBY RELEASE, WAIVE AND FOREVER DISCHARGE ANY AND ALL LIABILITY, CLAIMS, AND DEMANDS OF WHATEVER KIND OR NATURE AGAINST THE EVENT ORGANIZER, CROWDPASS AND ITS AFFILIATED PARTNERS AND SPONSORS, INCLUDING IN EACH CASE, WITHOUT LIMITATION, THEIR DIRECTORS, OFFICERS, EMPLOYEES, VOLUNTEERS, AND AGENTS (THE “RELEASED PARTIES”), EITHER IN LAW OR IN EQUITY, TO THE FULLEST EXTENT PERMISSIBLE BY LAW, INCLUDING BUT NOT LIMITED TO DAMAGES OR LOSSES CAUSED BY THE NEGLIGENCE, FAULT OR CONDUCT OF ANY KIND ON THE PART OF THE RELEASED PARTIES, INCLUDING BUT NOT LIMITED TO DEATH, BODILY INJURY, ILLNESS, ECONOMIC LOSS OR OUT OF POCKET EXPENSES, OR LOSS OR DAMAGE TO PROPERTY, WHICH I, MY HEIRS, ASSIGNEES, NEXT OF KIN AND/OR LEGALLY APPOINTED OR DESIGNATED REPRESENTATIVES, MAY HAVE OR WHICH MAY HEREINAFTER ACCRUE ON MY BEHALF, WHICH ARISE OR MAY HEREAFTER ARISE FROM MY PARTICIPATION WITH THE ACTIVITY.

4. This Release constitutes the sole and entire agreement of CrowdPass and me with respect to the subject matter contained herein and supersedes all prior and contemporaneous understandings, agreements, representations, and warranties, both written and oral, with respect to such subject matter. If any term or provision of this Release is invalid, illegal, or unenforceable in any jurisdiction, such invalidity, illegality, or unenforceability shall not affect any other term or provision of this Release or invalidate or render unenforceable such term or provision in any other jurisdiction. This Release is binding on and shall inure to the benefit of CrowdPass and the event organizer and our respective successors and assigns. All matters arising out of or relating to this Release shall be governed by and construed in accordance with the internal laws of the State of New York without giving effect to any choice or conflict of law provision or rule. Any claim or cause of action arising under this Release may be brought only in the federal and state courts located in New York, New York and I hereby consent to the exclusive jurisdiction of such courts.
